Behavioral Health Services
Community First Health Centers Behavioral Health Program is a counseling and resource program that provides a unique approach to coordinating primary medical care with behavioral health services. We know that behavioral health concerns can often complicate medical care; our integrated approach allows us to form a team with your provider to help improve your medical care and well-being.
Our goal is to achieve overall patient wellness by offering services designed to promote recovery, resilience, and improved quality of life. Our program will work with you to identify your specific needs and develop strategies to assist you in effectively managing your stress and coping with whatever challenges you may face.
Available Services
Once your primary care provider refers you to the behavioral health program, you will be eligible to see a Behavioral Health Specialist who will maximize your overall wellness by:
- Integrating your behavioral health needs with your medical care
- Completing screenings, referrals, and assessments
- Providing individual and/or group counseling

After-Hours Crisis:
If you are experiencing a behavioral health crisis after hours, please call your county’s Community Mental Health Access Center. For emergencies, please dial 9-1-1 or go to the nearest Emergency Department.

Eligibility
You are eligible to receive care through our Behavioral Health program if you are seeing a Community First Health Centers provider - just ask your doctor for a referral.
Most insurance plans are accepted, but your individual costs will be determined by your insurance company. We also take pride in offering this program to clients without insurance through our Sliding Fee program, so you can get the care you need at a price you can afford. You can direct all questions regarding eligibility to the Patient Navigator.

You can also access and complete the paperwork in our office, prior to your appointment. We suggest you arrive 30 minutes before your scheduled appointment time to complete the paperwork.
