Community First Health Centers is monitoring the current state of COVID-19 in our communities and emergency orders from local and state governments. We are open and here to provide you assistance through this uncertain time. If you have questions about COVID-19, your scheduled medical appointment or any of our other services we encourage you to call us. We have limited our hours, however our providers are available through our after hours service should you need us.
A few notes about our services during this time:
Medical Clinics (Algonac, New Haven and Port Huron): Our medical clinics are open Monday through Friday from 8:30 a.m. to 5:00 p.m. We have put in additional measures to keep our team and patients safe during this time. If you have symptoms consistent with COVID-19 (please see above) call before coming into the office, our providers may be able to help you remotely to prevent additional stress on you as well as our team and the community. We are currently only seeing sick patients in our office; we will utilize telehealth services for all other visits. Please contact your local Community First Health Center for additional information. We also have our after-hours phone line which will connect you with our provider on-call if you have urgent medical needs after we are closed. Again, we encourage you to call us should you have any questions related to your medical care or COVID-19.
Dental Clinic: Our dental clinic is currently closed. If you have a dental emergency, our dental team may still be able to assist you. Please call our office at 586-749-8002 and leave a voice message regarding your emergency. Our dental team will contact you regarding possible treatment options. We remain committed to providing our patients with superior care, even through this uncertain time, when possible we will provide emergency treatment to ensure your oral health needs are met.
Behavioral Health: Our behavioral health services remain available. We are currently using telehealth visits for the safety of our behavioral health providers and patients. Please contact the medical center you regularly visit to schedule your next behavioral health appointment.
Maternal Infant Health Program (MIHP): Our MIHP team is working remotely with enrolled clients. If you have questions about your MIHP visits or MIHP, we encourage you to reach out by phone at 586-749-5173 ext. 218.
WIC Services: There are many changes being made to the WIC program on a state and national level to ensure those using WIC are able to continue using this valuable service. Our team is making all needed adjustments to continue to serve our clients through these updates and changes, and help you navigate these changes. WIC clients are encouraged to contact our team by phone to discuss questions, concerns and new procedures. You can call our team Monday - Friday from 8:30 a.m. to 5:00 p.m. at 586-749-5173 ext. 232.
Homeless Health Care Program: If you are a patient using this Community First Health Centers program, please contact the medical center office you routinely see a provider at for assistance at this time.

Community-Based Health Care Services
Community First Health Centers has sought to make a positive impact on Michigan’s St. Clair and Macomb Counties for over 40 years. We provide community-based health and human services that promote the physical, mental, and emotional health of all members in the community.
Since 1975, Community First Health Centers has provided quality medical, behavioral, and educational health and support services to the communities of St. Clair and Northern Macomb Counties, assisted by private and public funding sources. We are proud to bring all of your health care needs into one National Committee for Quality Assurance (NCQA) Recognized Patient-Centered Medical Home Practice by offering all of the following services: Primary Health Care, Affordable Care, Transportation Services, Homeless Health Care, Behavioral Health Care, Lab & X-Ray Services, Maternal Infant Health Program, and WIC Services. When needed, language translation (including sign language and methods for communication with the hard of hearing and deaf) are utilized during appointments and care staff interactions.
This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
