Community First Health Centers is monitoring the current state of COVID-19 in our communities and emergency orders from local and state governments. We are open and here to provide assistance through this uncertain time. If you have questions about COVID-19, your scheduled medical appointment or any of our other services we encourage you to call us.
A few notes about our services during this time:
Medical Clinics (Algonac, New Haven and Port Huron): Our medical clinics are open and seeing patients, as well as accepting new patients. You do have the option to be seen in the office or utilize our telehealth services. Whether you need routine health care or are ill, even if it's a back to school physical or to update your immunizations, our team has safely prepared our offices for your return. Dental Clinic: Our dental team is currently seeing patients and rescheduling patients who had appointments canceled. Please note that patients are being prioritized by need, which means routine dental cleanings may be scheduled further out. We continue to add appointments daily, please reach out if you need routine dental care or have an emergent dental need. If you have questions, please call our dental team at: 586-749-8002.
Behavioral Health: Our behavioral health services remain available. We are currently using telehealth visits for the safety of our behavioral health providers and patients. Please contact the medical center you regularly visit to schedule your next behavioral health appointment.
Maternal Infant Health Program (MIHP): Our MIHP team is working remotely with enrolled clients. If you have questions about your MIHP visits or MIHP, we encourage you to reach out by phone at 586-749-5173 ext. 218.
WIC Services: There are many changes being made to the WIC program on a state and national level to ensure those using WIC are able to continue using this valuable service. WIC staff is seeing all clients virtually at this time. If you have questions about your WIC services or would like to learn more about qualifying call our team at: 586-749-5173 ext. 232.
Homeless Health Care Program: While our team is not out in the field at this time, they are still available to help those who may qualify for services through this program. Contact our office for more information: 586-552-8333.

Community-Based Health Care Services
Community First Health Centers has sought to make a positive impact on Michigan’s St. Clair and Macomb Counties for over 40 years. We provide community-based health and human services that promote the physical, mental, and emotional health of all members in the community.
Since 1975, Community First Health Centers has provided quality medical, behavioral, and educational health and support services to the communities of St. Clair and Northern Macomb Counties, assisted by private and public funding sources. We are proud to bring all of your health care needs into one National Committee for Quality Assurance (NCQA) Recognized Patient-Centered Medical Home Practice by offering all of the following services: Primary Health Care, Affordable Care, Transportation Services, Homeless Health Care, Behavioral Health Care, Lab & X-Ray Services, Maternal Infant Health Program, and WIC Services. When needed, language translation (including sign language and methods for communication with the hard of hearing and deaf) are utilized during appointments and care staff interactions.
This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
