Insurance

Community First Health Centers is proud to accept most commercial insurance plans, ALL forms of Medicaid, and patients with no insurance.  Please call your local Community First Health Centers office today to discuss your insurance plan or to enroll in our Sliding Fee Program!

If your insurance plan requires a co-payment, you will need to pay this at the time of your visit.

Referral Policies and Information

We will refer you to any specialist that is appropriate for our medical situation.

If your provider determines you require a referral to another provider, and your HMO/Managed care plan requires a documented referral, you will be referred to one of our Care Continuum Specialists to coordinate your referral.  He/She will work with your insurance company to ensure that your visit with the specialist is covered before your appointment.

To initiate a new referral or to check on the status of a referral, please contact your local Community First Health Centers office.

Emergencies

In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room that you are a patient of Community First Health Centers, and they will contact us.

After Hours

If you have an urgent medical need and want to talk to one of our doctors after regular hours, please call your local Community First Health Centers office.  A doctor is on-call 24 hours a day.  Your call will be returned promptly.
