Community First Health Centers

Maternal Infant Health Program

The Maternal Infant Health Program (MIHP) is a home visiting program for qualifying pregnant women and infants.  MIHP provides information and support on having a healthy pregnancy and baby.  Services include help with prenatal care, family planning, infant development, and community referrals.  We will work closely with your doctor to provide you with the best possible care.

Guidelines

If you are a Medicaid eligible pregnant woman or have a Medicaid eligible infant, you can participate in the program.  Medicaid will cover all of the costs associated with MIHP.  You will receive all of the services this program offers without any cost to you.

Home & Office Visits

We know it isn’t always easy to get around when you are expecting or have a new baby, so let us come to you! Many of our Maternal Infant Health Program clients like us to come to their home. Our team of registered nurses and licensed social workers all perform home visits and will schedule a time that is convenient for you. If you have an appointment with a Community First Health Centers provider or with our WIC services, we can schedule your MIHP appointments in our offices around your other appointments.

During our visits, we will provide you with information on the following:

  • Having a healthy pregnancy
  • Labor and delivery
  • Breastfeeding
  • Family planning options
  • Nutritional counseling
  • Emotional needs
  • Smoking cessation
  • Childhood development
  • Parenting
  • Community referrals

Medicaid Enrollment Assistance is available on site through our Algonac, Port Huron, and New Haven facilities' eligibility specialists. Depending on your needs, you may want to apply for one of these programs:

  • Healthy Kids is a Medicaid health coverage program for low-income children under 19 and pregnant women of any age. This program covers all prenatal care and hospital charges for pregnant women. Newborn infants can be enrolled in the Healthy Kids program at birth. Healthy Kids provides a comprehensive package of health care benefits, including vision, dental, and mental health services. Healthy Kids eligibility is based on your income.
  • MIChild is a low-cost health coverage program for low income uninsured children under the age of 19. MIChild has a higher income limit than Healthy Kids. There is a $10 per family monthly premium for MIChild. The $10 monthly premium is for all of the children in one family. Enrollees receive a comprehensive package of health care benefits including vision, dental, and mental health services.
  • MOMS (Maternity Outpatient Medical Services) Program covers prenatal care for pregnant women. The goal of the MOMS program is to provide immediate health coverage for pregnant women or recently pregnant women who are who are eligible for Emergency Services Only (ESO) Medicaid. MOMS provides coverage for outpatient prenatal services and pregnancy-related postpartum services for two months after the pregnancy ends. Medicaid ESO covers labor and delivery services. The MOMS program is available to provide immediate prenatal care while a Medicaid application is pending. Other women who may be eligible for MOMS include teens who, because of confidentiality concerns, choose not to apply for Medicaid, and non-citizens who are only eligible for emergency services only. There are income guidelines for all persons except teens.
